Medical Director / Hospice

Transitions Care seeks a PRN Hospice Medical Director to provide clinical leadership and compassionate end-of-life care for patients with serious or terminal illness. In this physician role, you will conduct patient evaluations, certify hospice eligibility, and guide complex symptom and pain management while honoring patient goals and preferences. You’ll collaborate closely with an interdisciplinary team, support families through difficult decisions, ensure regulatory compliance, and model Transitions Care’s mission of dignity, respect, and holistic support.
