Responsibilities
- Provide PT evaluations and therapy for residents in a skilled nursing setting.
- Develop and implement individualized treatment plans to improve mobility, balance, gait, strength, and daily function.
- Reassess progress, modify plans as needed, and document outcomes in resident records per regulatory standards.
- Collaborate with nursing, rehabilitation, and therapy colleagues to coordinate care and support discharge planning.
- Maintain infection control, safety, and patient privacy in all documentation and interventions.

Requirements
- Active Physical Therapist license to practice in North Carolina.
- Experience in or comfort with skilled nursing facility or long-term care settings.
- Ability to perform comprehensive PT evaluations, develop treatment plans, implement interventions, and document progress.
- Availability for a 13-week travel assignment starting 08/10/2026 with guaranteed 35 hours per week.
- Strong communication skills and ability to work as part of a multidisciplinary team.
