Registered Nurse Care Manager

To support specialty populations, the full-time remote New Jersey RN Care Manager will perform care coordination duties, assess clinical needs, and implement interventions to ensure efficient utilization of benefits. Key responsibilities Assess member's clinical needs against established guidelines for specialty populations Implement high-quality, cost-effective care based on customized population models and clinical practice guidelines Collaborate with members, families, and healthcare teams to advocate for and coordinate care resources Required qualifications Current RN license or Bachelor's degree in social work, health, or behavioral science Graduate level degree in Social Work or Health and Behavioral Science preferred Minimum of three years' experience serving members with special needs Strong knowledge of case management standards and managed care principles Valid New Jersey State License and Insurance for certain positions
