LPN Part Time Profession: Nursing Specialty: Long Term Care (LTC)

Under the direction of licensed nursing personnel, performs direct and indirect designated resident care within the scope of practice as defined by certification. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served. Possesses the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements. Provides care needed by adult and geriatric patient groups. Meets resident's activities of daily living (ADLs) needs as outlined in the plan of care. Ensures a safe and comfortable environment while meeting hygiene, grooming, dressing, incontinence/catheter care, elimination/toileting needs, turning/positioning, and skin care needs. Completes documentation per protocol. Maintains resident's environment and safety as outlined in the plan of care. Responds promptly to call lights and alarms. Provides safety mats and low beds as needed, and uses restraints under the direction of nursing. Maintains cleanliness of the unit, preventing behavior incidents and falls, and monitoring for malfunctioning equipment. Completes documentation per protocol. Meets resident's nutritional needs as outlined in the plan of care. Ensures meal delivery, tray preparation, adaptive equipment use, and feeding as necessary. Honors resident's preferences and choices. Completes documentation per protocol. Meets resident's rehabilitative needs as outlined in the plan of care. Includes range of motion, ambulation, transfers, proper body alignment, and the use of assistive devices. Completes documentation per protocol. Reports resident's rights, refusals, and concerns to Charge and/or Supervising Nurse in a timely manner. Identifies and reports changes in resident physical, emotional, and psychosocial status to Charge Nurse promptly. Occupational exposure to blood borne pathogens is reasonably anticipated. Required tasks involve potential contact with blood, body fluids, or tissues.
