Work Environment

This position is entirely remote, allowing for flexibility in managing your schedule while you provide necessary support to clients. Standard work hours are Monday to Friday from 8:00 AM to 4:30 PM, but the role may require occasional evening or weekend hours to meet client needs. As a Social Worker, you will engage closely with clients via video conferencing and phone calls, ensuring consistent and empathetic communication. You may also be required to provide on-call support for urgent situations outside of regular hours.

Key Responsibilities

• Conduct comprehensive assessments of seniors' psychosocial needs, taking into account their emotional, mental, and social factors.
• Develop and implement personalized care plans in collaboration with clients and their families, addressing specific goals and preferences.
• Provide direct counseling and emotional support to clients and their families, assisting them through life transitions and challenges.
• Act as an advocate for clients' rights, guiding them in accessing necessary community resources and services.
• Work collaboratively with a multidisciplinary team, including healthcare professionals, to coordinate care and enhance client well-being.
• Monitor clients through regular check-ins, ensuring their well-being and addressing any emerging concerns.
• Facilitate effective communication among family members involved in the care process, helping to resolve conflicts when necessary.
• Educate clients and families about community resources, support groups, and programs designed to promote social engagement.
• Maintain thorough and accurate documentation of client assessments, care plans, and progress notes to ensure compliance and track outcomes.
• Stay current on social work practices, ethical standards, and legislative changes impacting service delivery.

This position specializes in serving the senior population, which includes individuals experiencing challenges related to aging, chronic health conditions, and social isolation. Your role will involve not only responding to their social and emotional needs but also promoting resource accessibility and enhancing their overall quality of life through proactive engagement.

Qualifications

• Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
• Minimum of two years of professional experience in a social work position, ideally with geriatric clients.
• A deep understanding of social work principles, community resources, and issues specifically impacting older adults.
• Strong assessment, counseling, and problem-solving abilities.
• Exceptional communication and interpersonal skills, essential for effective teamwork and client interaction.
• Proficiency in electronic health record (EHR) systems and relevant software applications.

Preferred Experience

• Experience in community health settings or geriatric care programs.

Compensation and Benefits

We offer a competitive salary ranging from $60,000 to $100,000 annually, commensurate with experience and qualifications.
