Case Manager Nurse The Registered Nurse (RN) Case Manager (CM) promotes patient-centered care by coordinating the plan of care for patients’ stays, managing length of stay, ensuring appropriate resource management, and developing safe, appropriate discharge plans in collaboration with the multidisciplinary team. The RN CM facilitates progression and transition of care using established criteria and supports quality outcomes and patient throughput while balancing optimal care with resource utilization. Responsibilities Provide case management services for both inpatient and observation patients as assigned. Identify patients at risk for adverse outcomes during transitions of care. Perform comprehensive assessments of psychosocial, medical, and discharge needs of patients and families, including appropriate resource assessment. Reassess clinical condition as indicated, considering readmission risk and developing mitigation strategies, including education on healthcare resources and preventative care. Coordinate the plan of care and drive the discharge plan by collaborating with the multidisciplinary team and the patient’s physician. Partner with Social Services to ensure post-acute medical needs and care levels are appropriate and refer to Social Services when psychosocial determinants of health risk factors are identified. Involve patients, families, or significant others in clarifying needs, expectations, and realistic goals. Evaluate progression of care using evidence-based tools and approved criteria (InterQual) and report issues through the established chain of command. Make appropriate referrals to third-party payers and case management programs for chronic disease states. Facilitate patient throughput with a focus on effective care transition, quality, and efficiency. Document professional recommendations, discharge plans, care coordination interventions, and case management activities to communicate with all members of the healthcare team. Align patient needs with available resources to ensure a safe discharge/transition. Practice and adhere to the hospital’s “Code of Conduct” and “Mission and Value Statement.” Qualifications RN license (required) Associate Degree in Nursing or Nursing Diploma (required) Bachelor’s Degree in Nursing (preferred) 2+ years’ experience in case management or 3+ years’ experience in clinical nursing (required) InterQual experience (preferred) Benefits Comprehensive benefits for medical, prescription drug, dental, vision, behavioral health, and telemedicine services.
