- Requires active NP or PA licensure/certification and DEA authority

- NP or PA with geriatric, long-term care, or complex patient care experience
- Comfortable managing chronic and acute conditions
- Strong documentation and interdisciplinary care coordination skills
- Experience with advanced illness and end-of-life discussions preferred
