RN/LPN Restorative Nurse Job Summary:

We are seeking a dedicated and compassionate RN or LPN Restorative Nurse to join our team at Pleasant Acres Nursing & Rehabilitation Center in York, Pennsylvania. RN/LPN Restorative Nurse Responsibilities:

• Provide restorative care to residents, including developing and implementing individualized care plans to promote recovery, independence, and well-being

• Conduct comprehensive assessments and evaluations to identify areas of improvement and develop strategies to address them

• Collaborate with the rehabilitation team to create individualized therapy plans, setting goals and objectives for residents' continued progress

• Implement and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes

• Maintain accurate and up-to-date records, including assessment results, care plans, and progress reports

• Educate and support residents, families, and caregivers on restorative care techniques and strategies

• Participate in quality improvement initiatives and contribute to a culture of excellence and continuous learning

RN LPN Restorative Nurse Requirements:

• Current RN or LPN Licensure in the State of Pennsylvania

• Restorative experience a must
