About the Role:

Join STARS Adult Medical Day Care Center as a Social Worker and make a meaningful impact in the lives of our clients. We are looking for compassionate professionals who are dedicated to providing support and guidance in a nurturing environment.

Responsibilities:

• Conduct comprehensive assessments to determine client needs and develop individualized care plans.
• Provide counseling and support to clients and their families to enhance overall well-being.
• Coordinate with healthcare providers to ensure holistic care for clients.
• Facilitate group and individual therapy sessions as needed.
• Maintain accurate and confidential client records in compliance with regulations.
• Advocate for clients' rights and access to necessary resources and services.
• Participate in team meetings to discuss client progress and care strategies.
• Stay updated on community resources and services to better assist clients.

Requirements:

• A degree in Social Work from an accredited program.
• Current state licensure as a Social Worker.
• Experience in a healthcare or social service setting preferred.
• Bilingual candidates are strongly encouraged to apply.
• Strong communication and interpersonal skills.
• Ability to work collaboratively in a team-oriented environment.
• Compassionate demeanor with a commitment to client-centered care.
• Proficient in Microsoft Office and electronic health record systems.
