Job Description
ROLE - SPECIALTY

Schedule:
Monday to Friday, 8 AM to 5 PM, with rotating on-call and weekend duties.

Assignment Details:
Contract Length: 13 weeks
Guaranteed Hours: Full-time

Requirements:
- RN License
- Experience in hospice or palliative care is preferred.

Roles & Responsibilities:
- Provide compassionate care to patients and families in a variety of settings.
- Assess patient needs and develop personalized care plans.
- Educate patients and caregivers on symptom management and goals of care.
- Collaborate with an interdisciplinary team to deliver holistic care.
- Serve as a primary point of contact for an assigned caseload of patients.
- Provide support and guidance to caregivers in managing care at home or facility settings.
