Job Summary

At VitalCaring, our team members transform lives and foster hope through genuine caring. As a Licensed Practical/Vocational Nurse, you will provide direct, compassionate nursing care to hospice patients under the supervision of a Registered Nurse. You play a critical role in managing comfort, supporting families, and ensuring every encounter reflects our valuestrustworthy, capable, compassionate, proactive, and called. This role is essential to delivering dignified, high-quality end-of-life care.

Essential Functions

• Provide direct patient care in accordance with the state Nurse Practice Act, including specialized duties related to end-of-life care.
• Implement current nursing practice following a comprehensive assessment and the established plan of care.
• Assess and educate patients and their families/caregivers on diagnosis, care needs, and symptom management.
• Use and prepare equipment and supplies safely and effectively, consistently adhering to aseptic technique.
• Deliver appropriate pain and symptom management; monitor patient response to treatments and medications.

Communication & Coordination

• Document all care accurately and timely in alignment with the plan of care.
• Participate in care coordination and promptly report needs requiring involvement from other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the RN and/or Clinical Care Manager.

Additional Responsibilities

• Participate in ongoing personal and professional development.
• Contribute to quality assessment and performance improvement (QAPI) activities.
• Perform other duties as assigned by the Registered Nurse.

Qualifications

• Graduate of an accredited practical nursing or vocational nursing program.
• Minimum of three years of nursing experience preferred; home health, community-based, or medical-surgical experience ideal.
• Current state LPN/LVN licensure.
• Current CPR certification.
• Demonstrated adherence to professional nursing standards and practices.
• Licensed driver with a reliable, insured automobile meeting organizational requirements.
