We are seeking an experienced and compassionate Licensed Practical Nurse (LPN) to join our evening shift team at Rock River Nursing & Rehab in Janesville, WI, USA. As a member of our dedicated healthcare team, you will play a vital role in providing exceptional care and support to our residents.

About the Role:

As an LPN on our evening shift, you will be responsible for delivering high-quality nursing care to our residents, including administering medications, performing vital signs, and assisting with daily living activities. You will work closely with our interdisciplinary team to develop and implement individualized care plans, ensuring our residents receive the best possible care.

Responsibilities:

• Provide direct patient care to a diverse population of residents, including assistance with activities of daily living (ADLs) and skilled nursing interventions

• Administer medications and treatments as prescribed by physicians

• Perform vital signs, monitor vital functions, and report changes to the healthcare team

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Maintain accurate and detailed records of patient care and treatments

• Participate in quality improvement initiatives and performance improvement activities

• Collaborate with colleagues to ensure effective communication and coordination of care

Requirements:

• Current LPN licensure in the state of Wisconsin

• Experience working in a long-term care setting preferred

• Ability to work effectively in a fast-paced environment

• Strong communication and interpersonal skills

• Ability to maintain confidentiality and handle sensitive information

• Possess physical stamina to perform job responsibilities
