Job Summary

We are seeking a dedicated and compassionate Occupational Therapist to join our healthcare team. The ideal candidate will provide comprehensive therapy services to diverse patient populations, including pediatrics, geriatrics, and individuals with disabilities. This role involves developing personalized care plans, assisting with activities of daily living (ADLs), and supporting patients' functional independence across various settings such as inpatient, outpatient, home health, skilled nursing facilities, and schools. The Occupational Therapist will collaborate with multidisciplinary teams to ensure optimal patient outcomes and quality of care.

Duties

• Assess patients’ physical, cognitive, and emotional needs to develop individualized treatment plans.
• Assist patients with activities of daily living (ADLs) to promote independence and improve quality of life.
• Implement therapeutic interventions tailored to specific populations, including pediatrics, geriatrics, individuals with autism, and those in post-acute or acute care settings.
• Coordinate discharge planning and case management to ensure seamless transitions across care settings.
• Work with patients in various environments such as inpatient units, outpatient clinics, home health visits, schools, and skilled nursing facilities.
• Collaborate with healthcare professionals to monitor progress and adjust treatment plans accordingly.
• Educate patients and families on therapy techniques, adaptive equipment use, and safety measures.
• Maintain accurate documentation of patient assessments, treatment plans, progress notes, and discharge summaries in compliance with regulatory standards.
• Stay current with advancements in occupational therapy practices and participate in ongoing professional development.

Qualifications

• Valid state licensure or certification as an Occupational Therapist.
• Experience working with diverse populations including pediatrics (school-based or early intervention), geriatrics (geriatrics or skilled nursing), individuals with autism or disabilities, and post-acute care patients.
• Knowledge of ADLs assistance, discharge planning, case management, physiology, anatomy, medical terminology, and patient care protocols.
• Experience in acute care settings such as ICU or inpatient units is preferred.
• Familiarity with outpatient clinics, home health services, skilled nursing facilities, or school environments is highly desirable.
• Strong communication skills and ability to work collaboratively within multidisciplinary teams.
• Ability to adapt treatment approaches based on individual patient needs and care settings.
