With a renowned reputation for pulmonary care and decades of experience in ventilator weaning, we provide unparalleled quality in short-term rehabilitation therapy and long-term skilled nursing services. At Carter, we prioritize patient-centered care and clinical excellence across the continuum of long-term acute and skilled nursing care. Responsibilities Demonstrate a commitment to community health, evidence-based practice, academic medicine, and patient-centered care. Adhere to applicable regulations for physician services and medical management. Initiate referrals for consult and specialty services as needed. Deliver direct clinical care, including primary care, chronic ventilator management, and acute illness treatment, while following established protocols and evidence-based guidelines. Facilitate emergency and planned discharges to acute hospitals or medical treatment facilities. Ensure timely and accurate completion of medical records, orders, and documentation.
