Job Description

The position is responsible for the assessment, coordination, delivery, and evaluation of nursing care in an acute patient setting. This includes direct patient care, patient and family education, and transitions of care. The role calls for delivering holistic and individualized care to all patients within an assigned area.

The successful candidate will develop, implement, and coordinate a comprehensive care plan that accounts for psychological, sociocultural, spiritual, economic, and lifestyle factors. The position requires fostering and maintaining collaborative relationships with patients, their families, physicians, and other healthcare providers through timely and effective communication.

Principal Accountabilities

• Assessment and Planning of Patient Care:
  
  
  • Conduct comprehensive patient and family assessments.
  • Collaborate and communicate with healthcare team members to ensure a patient-centric plan of care.
  • Effectively streamline patient workflow to improve outcomes.
  • Communicate plans of care with co-workers and across shifts.
• Provision of Patient Care:
  
  
  • Deliver competent and compassionate patient care in line with established guidelines and standards.
  • Maintain a safe and comfortable therapeutic environment.
  • Delegate tasks appropriately based on team members' capabilities and legal scopes of practice.
  • Educate patients and families clearly, ensuring understanding and addressing their learning needs.
  • Ensure the confidentiality of patient health information (PHI).
• Evaluation of Patient Care:
  
  
  • Monitor patient care interventions and assess treatment outcomes.
  • Provide ongoing education for patients and families throughout their care journey.
  • Adjust care plans as necessary based on continuous evaluation and documentation.
  • Serve as a clinical mentor to new staff and assist in their orientation.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse (RN) licensure in California.
• Basic Life Support (BLS) certification.
• Advanced Cardiac Life Support (ACLS) certification preferred.
• National Institutes of Health Stroke Scale (NIHSS) certification.
• Minimum of 2 years of recent experience in Med-Surg/Telemetry.
• Experience with the EPIC charting system is required.

Skills and Knowledge

• Strong knowledge of nursing theories, concepts, and practices.
• Effective written and verbal communication skills.
• Time management and organizational skills for prioritizing patient care.
• Familiarity with healthcare industry regulations and best practices.
