Discharge & Care Coordination Specialist

St Bernards Medical Center

The discharge planner at St. Bernards Healthcare will coordinate services needed upon discharge, including sending referrals to appropriate facilities such as nursing homes and residential care facilities. The role also arranges transportation when appropriate and coordinates outpatient mental health services. Ideal candidates demonstrate experience with geriatric and adult populations and possess the ability to assess patient needs across the lifespan, aligning discharge plans with individual #J-18808-Ljbffr
