LVN / LPN - LTC/SNF

Job Summary:

Responsibilities:

• 
  Administer nursing care to residents in accordance with physician orders and care plans.
  
  
• 
  Monitor and document patient health status, changes, and responses to treatment.
  
  
• 
  Collaborate with interdisciplinary team members to develop and update care plans.
  
  
• 
  Ensure compliance with all licensing, safety, and procedural standards.
  
  
• 
  Assist residents with daily activities, medication administration, and treatments.
  
  
• 
  Maintain accurate and detailed patient records, reports, and documentation.
  
  

Qualifications:

• 
  Hold an active Hawaii Nursing License prior to start date.
  
  
• 
  Ability to work consecutive weekends as scheduled.
  
  
• 
  Demonstrate flexibility to rotating shifts and full-time hours.
  
  
• 
  Prior experience in LTC/SNF preferred.
  
  
• 
  Strong clinical assessment and documentation skills.
  
  
• 
  Excellent communication and teamwork abilities.
