RequirementsResponsibilitiesProvide comprehensive outpatient primary care to pediatric, adult, and geriatric patients. Perform preventive care, acute care visits, chronic disease management, and health screenings. Develop and maintain continuity of care for an assigned patient panel. Collaborate with nurses, behavioral health providers, pharmacists, care managers, and other interdisciplinary team members. Refer patients to specialty care and community resources as appropriate. Utilize evidence-based medicine and participate in quality improvement initiatives. Serve as a preceptor for residents and students as assigned. Maintain accurate and timely documentation in the electronic health record. Participate in departmental meetings and organizational initiatives.Education and Experience Required:· Possession of a license to practice as a medical doctor in New York State. Licenses and Certifications:· Board eligibility or Board certification.· Must obtain credit of CME as required. · Must meet privileging and credentialing requirements for Physicians. · Infection Control certificate. · BLS through the American Heart Association.
