Speech Language Pathologist (SLP) – Home Health | Per Diem
North Bergen, NJ | Serving Hudson County

We are currently seeking an experienced Speech Language Pathologist (SLP) to join our growing team on a per diem basis supporting adult and geriatric clients throughout Hudson County, NJ.

What You’ll Do

As a Home Health Speech Language Pathologist, you will:

• Provide in-home speech therapy evaluations and treatments for adult and geriatric clients
• Assess, diagnose, and treat speech, language, communication, cognitive, and swallowing disorders
• Develop individualized treatment plans in collaboration with physicians and care teams
• Modify care plans as patient needs evolve
• Educate patients and caregivers to promote safety, independence, and improved quality of life
• Collaborate closely with nurses, therapists, and interdisciplinary team members
• Complete timely electronic documentation using a tablet-based EMR system

Qualifications

• Current NJ Speech Language Pathologist license required
• Certificate of Clinical Competence (CCC-SLP) preferred
• Home health experience preferred but not required
• Strong communication and organizational skills
• Comfortable with electronic documentation/EMR systems

Additional Information:

Base Pay, depending upon qualifications and subject to negotiation:

$65.00 - $75.00 per point
