Your day-to-day responsibilities will include coordinating treatment plans, connecting patients to resources, and ensuring continuity of care. You will work collaboratively with a multidisciplinary team to advocate for patients, facilitate safe discharge planning, and monitor progress. This position is ideal for someone with strong behavioral health assessment skills and a passion for patient-centered care in a rural healthcare environment.
