AIDS Healthcare Foundation seeks an RN Medical Care Manager in Beverly Hills to deliver comprehensive medical case management under the Ryan White program. You will assess patient needs, educate patients about diagnoses and meds, and coordinate access to services within the healthcare center. You will develop Health Risk/Needs Assessments and Service/Care Plans, refer to housing, food, and other supports, and work with providers to move plans into action toward viral suppression and improved #J-18808-Ljbffr AIDS Healthcare Foundation
