Responsibilities

• Perform comprehensive speech-language pathology evaluations for residents, including swallowing and communication assessments as appropriate
• Develop, implement, and monitor individualized treatment plans and therapeutic activities
• Provide therapy for voice, fluency, language, cognition, and swallowing disorders according to residents’ needs
• Document assessments, treatment plans, progress notes, and outcomes in the resident’s chart per facility policy
• Collaborate with the interdisciplinary rehab team to coordinate care, discharge planning, and family education
• Participate in quality improvement activities and document patient outcomes to support program goals
