Job Description Assess patients to develop and initiate treatment plans based on the evaluation results Review the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment required Provide direct patient care by established protocols and patient care guidelines Document findings, progress, and instructions to patients and caregivers Responsible for patient care performed by assistants and rehabilitation technicians Complete a written plan of care, develop goals based on evaluation findings, and develop a comprehensive program to attain goals Implement physical therapy treatment program and provide essential physical therapy treatments Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies Ensure timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports Qualifications Graduate from an accredited college with an APTA curriculum New or recent graduates are welcomed and encouraged to apply Additional Information What We Offer: Medical and Dental insurance Generous Paid Time Off (PTO) 6 additional paid holidays annually Bonus Rewards Program Employee discount plans Employee Assistance Program (EAP) Community and Team Engagement Partnership with local high school athletic teams Community engagement through volunteering, mission trips, and philanthropy Annual team kick-off event and other team-building activities Professional Development Continuing education allowance Company sponsored in-services Leadership and Clinical Director Training Sports Residency Program Multiple opportunities for professional development, specialization, and leadership!
