Submit a color photo taken in past 6 months with plain background, no selfies or filters. Primary Source Verification of professional license and/or certification within 30 days of start date and updated upon expiration. Documentation of two measles vaccines, two mumps vaccines, and one rubella vaccine or two MMR vaccines, or proof of immunity via blood test. Strongly preferred Hepatitis B vaccination or proof of immunity or declination. Documentation of TB skin test screening within 3 months of start date and updated if >6 month break, or negative QuantiFERON TB Test or T-spot within 3 months of start date and updated if >6 month break. Physician Health Statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break. MGB Criminal Background Check within 45 days prior to start date and updated if >6 month break. MGB Massachusetts CORI background check within 30 days prior to start date and updated if >6 month break. Include past 7 years of work experience with city and state for submission. Two references within 4 years of submission, minimum one supervisor or above in skillset. MGB Skills Checklist must be completed within 1 year of submission. MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions upon acceptance of assignment. Verified highest level education achieved through background check company. MGB SSN Trace must be run with criminal check listing addresses/aliases for past 7 years. MGB National Sex Offender Check run within 30 days of start and updated annually. MGB Exclusion Checks run within 30 days of start and annually. MGB Candidate Submission Coversheet required for submission to initial assignment and new facility.
