Shift Details: Various locations and shifts. Licensed Practical Nurse (LPN) Duties & Responsibilities: Provide direct patient care under the supervision of a Registered Nurse or physician. Administer medications and treatments as prescribed. Monitor and record patients' vital signs and report changes in condition. Assist with patient assessments and implement individualized care plans. Perform wound care, specimen collection, and other basic nursing procedures. Document patient care accurately in the electronic medical record (EMR). Educate patients and families on health maintenance and treatment plans. Assist with admissions, discharges, and transfers. Maintain a safe, clean, and therapeutic environment for patients. Follow facility policies, infection control protocols, and regulatory standards. Minimum Qualifications Education and Experience requirements at this level consist of possession of a valid permanent Oklahoma license as a licensed practical nurse as approved by the Oklahoma Board of Nursing plus one year of practical nursing experience; or Licensed Practical Nurse License recognized by the Enhanced Nurse Licensure Compact (eNLC) plus one year of practical nursing experience Minimum of 1 year of recent experience within the last 3 years as a LPN. Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required. Copy of all current required certifications and compliance documents (e.g., .BLS, N95 fit test, immunization records, TB test, or other client‑specific requirements).
