AA/EOE Job Category Day Habilitation Responsibilities The Day Habilitation Case Manager is responsible for assisting in the development and growth of independence of participants through implementation of the following therapeutic services: independent/self-help development, sensorimotor development, communication development and behavior development. Additional therapeutic services may include, but are not limited to: social development, independent living development and affective development. These specialized services will be carried out through the implementation of individualized service plans, goals, curriculum, interventions, consultative therapies and other scheduled program activities in a variety of settings. The Case Manager is responsible for enhancing participants’ independence, dignity, safety, and personal and self-care activities. This is done with the core principal of person-centered input and choice. In all programs, human rights and responsibilities are a foundation of the agency. Qualifications B.S. in Psychology, Special Education or related field and one year experience. In lieu of a college degree, a High School diploma or GED with 2 plus years paid work experience in an instructor/care management role with developmental disabilities population. Commitment to community inclusion and upholding the dignity of people with disabilities, flexibility and responsiveness to a variety of requests. Ability and willingness to learn ASL, non-violent restraint, CPR/First Aid. Good time management, organizational and computer skills. Ability to meet deadlines, production and paperwork requirements. Must have strong oral and written communication skills with participants, program staff, families, therapists and other service providers/monitors. Capacity to learn/perform basic sign and other augmentative communication systems essential. Must be computer literate. Additional Info Work Schedule: 40 hour work week; Monday – Friday 8:00 AM – 4:00 PM plus flexible hours for meetings.
