Key Responsibilities:

• Conduct in-home visits to assess safety, monitor health conditions, and provide clinical coaching, including medication reviews.
• Follow structured care plans and offer ongoing support via a digital platform, including referrals and health coaching.
• Build supportive, trusting relationships with family caregivers to drive positive health outcomes and prevent burnout.
• Maintain timely and accurate documentation in compliance with regulatory standards.
• Participate in special projects and team initiatives as needed.

Qualifications:

• Active Registered Nurse (RN) license in Massachusetts.
• Minimum of 2 years' recent experience in eldercare or supporting adults with disabilities.
• Valid driver's license and auto insurance; ability to travel for in-home visits.
• Strong background in case management, care planning, and assessments in complex care settings.
• Proficiency with MS Office and case management software.
• Bilingual candidates are encouraged to applylanguage skills may positively impact compensation.
