About the Role We're looking for a mission-driven Rheumatologist to join our growing specialty care team, delivering high-quality care to patients with complex, chronic musculoskeletal and autoimmune conditions. This role is structured to evolve over time as physicians build experience within AndHealth's care model. All Rheumatologists begin in a Shift-Based Specialist role that combines direct patient care with dedicated time for longitudinal patient management, multidisciplinary collaboration, and panel development activities. This structure allows physicians to contribute immediately to patient access while beginning to build the patient relationships, care coordination workflows, and clinical leadership capabilities that define long-term success within the model. As physicians gain experience within the model and demonstrate readiness for longitudinal panel ownership, they transition into the Panel Manager role with increasing accountability for patient outcomes, treatment strategy, and clinical leadership across their panel and care team. We've intentionally designed this progression to create a sustainable and rewarding physician experience: one that allows physicians to initially focus on direct specialty care while evolving toward deeper patient relationships, greater clinical impact, autonomy, and expanded compensation opportunity over time. Day to day, the role is focused on the clinical work that only a physician can do: complex evaluations, treatment strategy, specialist consultations, and oversight of escalated cases. Our multidisciplinary team handles much of the administrative and coordinative work traditionally placed on specialists. Prior authorizations are managed by our team, not you. Routine follow-up and medication management are supported by Clinical Pharmacists. Care coordination across our Community Health Center partners is handled by Care Navigators. This position reports to the Chief Medical Officer. What Makes This Role Different Most rheumatology practices are built around throughput. Ours isn't. Our care model is designed around multidisciplinary specialty care teams that allow physicians to practice at the top of their license. You'll work alongside Advanced Practice Providers, Clinical Pharmacists, Care Navigators, and Health Coaches who collectively support the patient journey and reduce administrative burden on physicians. Your caseload is focused on patients with complex inflammatory arthritis and systemic autoimmune disease, diagnostically challenging presentations, and cases where physician-level expertise has significant impact. A typical schedule is structured around new patient evaluations, specialist consultations, and escalated cases rather than high-volume routine visits and RVU-driven throughput. Our hybrid care model combines virtual care with periodic in-person sessions at Community Health Center partner sites, allowing physicians to build meaningful patient relationships while expanding access to specialty care in underserved communities. Those partnerships matter. Many of our patients have waited months to see a rheumatologist. This is an opportunity to deliver specialty care to patients who otherwise may not have access to it.What You'll Do Clinical Responsibilities
- Conduct new patient evaluations and specialist consultations for patients with complex rheumatologic and autoimmune conditions, including inflammatory arthritis and systemic autoimmune disorders.
- Diagnose, treat, and manage chronic rheumatologic conditions across both virtual and in-person care environments.
- Provide physician-level review and oversight of escalated clinical cases from APPs, Clinical Pharmacists, and care team partners.
- Develop individualized treatment plans integrating medical therapy, lifestyle interventions, and whole-person care approaches.
- Educate patients on disease processes, treatment pathways, and long-term management strategies.
- Identify co-occurring or contributing conditions that may impact diagnosis, treatment response, or patient outcomes.
- Participate in collaborative case reviews and interdisciplinary care planning.
- Maintain compliance with all CHC, clinical, and regulatory requirements.

- Collaborate closely with APPs, Clinical Pharmacists, Care Navigators, and Health Coaches to deliver coordinated patient care.
- Provide clinical guidance, mentorship, and support to APPs and pharmacy partners.
- Contribute feedback on workflows, care protocols, and patient experience initiatives.
- Support a culture of clinical excellence, accountability, continuous improvement, and patient-centered care.
- Expanded performance-based compensation tied to patient outcomes and panel performance.
- Greater clinical autonomy and accountability for longitudinal patient outcomes rather than visit-level productivity.
- Leadership within the multidisciplinary care team, including mentorship and guidance for APPs and pharmacy partners.
- Clinical program development, including protocol design, quality initiatives, and service line evolution.
- Relationship-building with Community Health Centers and health system partners.
- Dedicated care team infrastructure that scales alongside panel growth, including pharmacist and care coordination support.
Education & Licensure Requirements:
- M.D. or D.O. from an accredited medical school required
- Board Certification in Rheumatology required
- Active, unrestricted medical license required; Ohio licensure or willingness to obtain required
- Completion of an ACGME-accredited Internal Medicine residency and Rheumatology fellowship required
- DEA certification required
- Basic Life Support (BLS) certification required
- Experience with chronic, complex rheumatology patients and autoimmune conditions preferred
- Experience training or mentoring APPs preferred
- Comfort practicing in virtual and in-person care environments
- Strong written and verbal communication skills; ability to explain complex concepts clearly to patients and care teams
- Comfort working with patients from diverse backgrounds and varying levels of healthcare literacy
- Proficiency with EHRs and healthcare technology platforms
- Ability to work independently while contributing to a collaborative, team-based care model
- Belief in a whole-person approach to chronic disease management
- Willingness to travel periodically to partner sites as needed
