Nurse Case Manager

Providing telephonic care management for members with complex and chronic needs, the full-time Multi-State Licensed Nurse Case Manager will assess, develop, implement, coordinate, and evaluate care plans while working remotely with occasional in-person training sessions. Key responsibilities Conduct assessments to identify individual needs and develop specific care management plans Implement care plans by facilitating authorizations and coordinating resources Monitor and evaluate the effectiveness of care management plans, making modifications as necessary Required qualifications BA/BS in a health-related field and a minimum of 3 years of clinical experience Current unrestricted RN license in applicable state(s) Multi-state licensure required for servicing members in different states
