At LCS, Sagewood, and Acacia, we create living experiences that truly enhance the lives of seniors./ppstrongWhat youll do:/strong/pp Helps with the maintenance and accuracy of records in regard to patient care for all patients on personal case load./pp Responsible for the correct reporting of billing information of Physical Therapy in regard to all patients on personal case load./pp Assists the Physical Therapist in carrying out procedures and programs./pp Performs routine treatment procedures in accordance with the planned program./pp Contributes to program planning and development in collaboration with a PT./pp Observes, records and reports to their supervisor conditions, reactions and responses related to their assigned duties./pp Reports any pertinent findings to the registered therapist./pp Participate in development and presentation of in-services for the facility as requested./pp Perform clinical functions: screens, following of treatment plans, and delivery of treatment in accordance with facility, Medicare, and policies and procedures./pp Terminates services formulating discontinuation and follow-up plans when maximum benefit is received under the supervision of a PT./pp Assist and assess transfers, gait and balance as required by functional mobility of patient level of assistance from minimum to maximum./pp Participate in meetings regarding patient care as appropriate./pp Assist when appropriate with any community education and public relations activities of the facility./pp Maintains a high standard of resident care./pp Responds immediately to all crisis and/or calls for assistance./pp Actively participates in in-services, department meetings and continuing education. Graduate from an approved and accredited therapist program. Sedentary tasks related to job approximately 10% Physical tasks as listed approximately 90%. CPR certified.
