Advanced Practitioner Wound Care - Part-Time

Job Summary Under general supervision, responsible for the clinical management of patients in accordance with hospital, state, and national regulatory agency standards. Qualifications Education: Physician Assistant Program or Certified Nurse Practitioner Program. Experience: Two (2) year experience as an adult advanced practitioner. Preferred clinical experience in Wound Care, Lymphedema, and Hyperbaric Therapy. Licensure: Licensed as an Adult Nurse Practitioner or Physician’s Assistant by the Maryland Board of Medical Examiners / Maryland State Board of Examiners of Nurses. Certification as required. Completion of “Healthcare Provider” CPR course. Skills Analytical skills to assess patients and implement appropriate intervention. Debridement of wounds as indicated. Application of therapeutic wound dressings as indicated. Leadership of a care team. Patient management on a weekly basis. Follow‑up of cultures and diagnostic studies. Responsibilities Assumes responsibility for the initial evaluation and treatment of outpatients at the Lymphedema Center, Wound Care Center, and/or Hyperbaric Oxygen Center. Provides consultation and treatment on admitted patients for wounds and hyperbaric oxygen therapy. Performs history and physical with appropriate documentation. Writes and updates orders in compliance with hospital policy. Evaluates patient progress, collaborates with attending physicians and staff, and alters treatment modalities. Evalues medical, social, and educational patient needs. Initiates referral to ancillary departments and community resources based on patient need, documenting progress in the note. Establishes medical diagnoses in compliance with standard accepted medical practice. Makes immediate referral of acute and/or life‑threatening conditions to attending physician, designee, or Emergency Department. Consults with supervising physician or other consultants for patients not improving as expected. Performs technical functions covered by the job description and delegation agreements, documents results, and monitors patient response. Participates in regular, interdisciplinary patient care rounds. Acts as a clinical resource for the Wound Care Center and Hyperbaric Oxygen Therapy. Provides a leadership role in meeting provider coverage for inpatients with lymphedema and/or wound care needs and triage them as a midlevel provider. Performs QA and other department‑level functions. Runs or participates in center care rounds. Participates in required education at all centers. Attends required staff meetings. Provides mentorship to nurses in the Wound Care Center, Hyperbaric Oxygen Therapy, and GBMC. Participates in competency education tasks such as presiding over outpatient center mock codes. Performs activities to meet patient and family education needs. Participates actively in health teaching of patients and families, including preventative health care and current medical conditions and treatment regimes. Plans and reviews home care instructions with patients and families as needed, including community resources and hospital ancillary services. Maintains continual communication with health-team members regarding education and discharge planning. Assumes responsibility for educational advancement by pursuing activities to meet identified educational needs, required certifications, and mandatory in‑service training.
