Licensed Practical Nurse – Home Health

Schedule: 8 hours per shift

Assignment Details:

• Contract Length: 14 weeks
• Guaranteed Hours: 40 per week

Requirements:

• Current Nursing License in the practicing state
• Valid Driver's License
• Current CPR Certification
• Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice
• Proficiency in using electronic medical charting and time-keeping software

Roles & Responsibilities:

• Administer skilled nursing care to patients in their place of residence as prescribed by the physician and in accordance with the established plan of care.
• Assist in providing nursing services and identifying the patient's physical, psycho-social, and environmental needs; report outcomes to the RN Case Manager.
• Participate in planning, coordinating, and implementing care with the RN to ensure quality, cost-effective patient care.
• Communicate effectively with patients, families, and the healthcare team; document outcomes of interventions accurately.
• Assist patients and families in learning appropriate self-care techniques and assess their responses to care.
• Utilize infection control measures to protect both staff and patients.
• Assist in specialized duties for end-of-life care for hospice patients.

Compliance / Notes:

• Proof of current Hepatitis consent/declination and TB vaccination record required.
• Must maintain good and regular attendance.
• Participation in annual in-service and performance improvement programs expected.
