Licensed Practical/Vocational Nurse (LPN/LVN) – Home HealthLocation: Long Beach Southeast and North Orange CountyJob Summary We're looking for a skilled and compassionate LPN/LVN to provide in-home nursing care under the supervision of a Registered Nurse (RN). You'll support patients with daily care needs, monitor their condition, and help ensure safe, high-quality home health services.ResponsibilitiesProvide nursing care according to the RN-developed Plan of Care.Monitor and document changes in patient condition; report to RN/physician as needed.Assist with activities of daily living to promote patient independence.Educate patients and caregivers within LPN/LVN scope of practice.Maintain accurate EMR documentation.Participate in case conferences and coordinate care with the home health team.Follow infection-control, HIPAA, and company policies.RequirementsCurrent LPN/LVN license in the state of practice.Graduate of an accredited practical/vocational nursing program.Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.Current CPR/BLS certification.Valid driver's license, reliable insured vehicle.Strong communication, clinical judgment, and ability to work independently.Physical RequirementsAbility to lift/transfer patients and perform physical caregiving tasks.Ability to pass required physical exams and annual PPD testing.Why Join us?We are committed to our employees' well-being, offering a comprehensive employee benefits package that includes, but is not limited to:· Competitive salary and comprehensive benefits package (medical, dental, vision, life, AD&D, disability, EAP, 401(K) with 4% match· Paid time off and paid holidays· Mileage reimbursement or company vehicle (role dependent), plus fuel card· Employee referral program and opportunities for growth and advancementThis position is based on a Pay per visit compensation model.Pay per visit ranges from $40 to $45 visit commensurate with experience and depending on type of visit.
