Licensed Practical Nurse (LPN) – Direct Care

Community Access, Inc. – Stillwater, Oklahoma

$29.00 per hour | $31.00 per hour for overnight and weekend hours

$1,500 SIGN-ON BONUS – Available when eligibility requirements and conditions are met!

Community Access, Inc. is a growing Oklahoma provider of services for individuals with intellectual and developmental disabilities (I/DD). We are looking for dependable and compassionate Licensed Practical Nurses (LPNs) to join our team in Stillwater, Oklahoma .

This is a direct-care nursing position where you will have the opportunity to provide individualized nursing care and support while developing meaningful relationships with the people you serve.

What You'll Do

As an LPN with Community Access, your responsibilities may include:

• Provide skilled nursing care and interventions
• Monitor and document individuals' physical and mental status
• Administer medications and treatments as ordered by authorized medical providers
• Document medications, treatments, nursing interventions, responses, adverse reactions, and changes in condition
• Implement written nursing plans of care
• Communicate changes and healthcare needs to appropriate members of the individual's support team
• Attend medical appointments as needed
• Participate in Individual Plan (IP) and interdisciplinary team meetings as requested
• Provide direct care and general support to individuals when skilled nursing interventions are not being performed
• Assist in maintaining a safe, supportive, and comfortable home environment
• Follow Community Access policies, procedures, training requirements, and applicable regulations

Minimum Requirements

Applicants must:

• Hold a current and valid Licensed Practical Nurse (LPN) license
• Be able to successfully complete required background checks
• Be dependable and able to report for scheduled shifts
• Have strong communication and documentation skills
• Be able to work effectively as part of an interdisciplinary team
• Be committed to treating individuals with dignity and respect
• Be willing and able to complete all required training
• Hold a current Oklahoma Drivers License
