Behavioral Health Clinician 1 or 2 (ASW or LCSW) - Wellness Recovery Services The Behavioral Health Clinician – Wellness Recovery Services (WRS) is a core clinical member of the WRS team at One Community Health. WRS is a specialty, year-long stepped-care behavioral health program serving patients with co-occurring mental health and substance use disorders within the Choice and Partnership Approach (CAPA) framework. The clinician provides individual, couples, and family psychotherapy, facilitates recovery-oriented group treatment, and serves as the co-occurring disorders specialist within a transdisciplinary care team. The clinician delivers goal-directed, episode-based treatment across the WRS stepped-care continuum, from pre-treatment engagement through active treatment and aftercare. In addition to ongoing psychotherapy responsibilities, the clinician conducts WRS Choice appointments, supporting collaborative service matching and treatment planning for patients entering specialty recovery services. The clinician works closely with Medication Assisted Treatment (MAT) providers, psychiatry, primary care, and case management to ensure integrated, whole-person care. This role requires strong clinical skills in co-occurring disorder treatment, fluency in moderation management and recovery-oriented models of care, and the ability to integrate clinical depth with medical collaboration in a fast-paced Federally Qualified Health Center (FQHC) environment. Essential Functions Psychotherapy & Clinical Treatment Provide individual, couples, and family psychotherapy for patients with co-occurring mental health and substance use disorders. Deliver goal-directed, episode-based treatment consistent with the WRS stepped-care model, including planned step-downs in session frequency and collaborative discharge planning. Apply evidence-informed modalities appropriate to co-occurring presentations (e.g., Motivational Interviewing, CBT, DBT-informed interventions, ACT, trauma-focused approaches). Integrate principles of moderation management throughout treatment, meeting patients at their current stage of readiness and celebrating incremental change. Conduct WRS Choice appointments as assigned, including collaborative assessment, level of care determination, and service matching. Group Treatment & Recovery Programming Facilitate process groups and psychoeducational groups rooted in the Living in Balance curriculum and other evidence-based SUD treatment models. Support recovery groups and events across the WRS continuum, including pre-treatment motivational and drop-in groups. Administer Freedom From Smoking or other evidence-based approaches to smoking and tobacco cessation. Contribute to the ongoing development and refinement of group curriculum and recovery programming. Co-Occurring & Substance Use Disorder Care Complete substance use and co-occurring assessments, including risk screening (suicide, self-harm, overdose risk, withdrawal risk, psychosis) and use of standardized tools (PHQ-9, GAD-7, AUDIT, DAST). Utilize ASAM criteria to inform level of care determinations and facilitate referral to higher levels of care when clinically indicated. Support patients experiencing return-to-use with non-judgmental re-engagement, safety planning, and treatment plan adjustment. Provide overdose prevention education, including naloxone education and fentanyl test strip information, regardless of the patient's stage of readiness. Medication Assisted Treatment (MAT) Collaboration Understand and support patients receiving Medication Assisted Treatment, including buprenorphine and naltrexone. Collaborate with MAT prescribers, nursing, and behavioral health staff to coordinate integrated treatment for patients with opioid and alcohol use disorders. Reinforce medication adherence, address ambivalence, and integrate MAT-related content into psychotherapy and group treatment. Maintain familiarity with 42 CFR Part 2 confidentiality requirements governing substance use disorder treatment records. Transdisciplinary Collaboration & Care Coordination Collaborate with transdisciplinary teams across primary care, psychiatry, MAT, case management, peer support, and external community providers. Facilitate warm handoffs and transitions of care across the WRS continuum and between service lines. Address barriers to engagement, including housing instability, transportation, chronic medical conditions, and psychosocial stressors. Participate in WRS case reviews, clinical supervision, interdisciplinary case conferences, and team huddles. Documentation, Quality & Compliance Adhere to department documentation standards, including timely completion of progress notes, Collaborative Care Plans, and discharge summaries in Epic. Complete required AOD consents and program enrollment documentation for WRS patients. Support quality assurance metrics and grant requirements (e.g., SAMHSA, Ryan White, Medi-Cal, SOR). Contribute to program development initiatives and workflow optimization within Wellness Recovery Services. Minimum Requirements Master's degree in social work (MSW) from an accredited institution. Active Registration as an Associate Social Worker (ASW) or Licensed Clinical Social Worker (LCSW) license in good standing. Training and experience working with substance use disorders and co-occurring conditions. Demonstrated experience conducting behavioral health and substance use assessments. Experience providing individual and group psychotherapy. Experience in integrated care, community mental health, or healthcare settings. Strong risk assessment and clinical formulation skills. Ability to function effectively in a multidisciplinary, fast-paced environment. Excellent clinical documentation and communication skills. Commitment to providing care grounded in compassion, service, harm reduction, and cultural humility. Active CPR and BLS Certification. Preferred Background 2 years of post-LCSW licensure experience. Certification or specialized training in addiction treatment (e.g., CADC, certification in co-occurring disorders treatment). Lived experience with substance use disorder recovery and or Medication Assisted Treatment, and the ability to draw on that experience appropriately to strengthen engagement, reduce stigma, and support patient-centered care. Experience delivering the Living in Balance curriculum or comparable structured SUD treatment programming. Experience supporting patients receiving Medication Assisted Treatment. Training in Motivational Interviewing, trauma-informed care, or family systems approaches. Familiarity with ASAM criteria and level of care determination. Experience with tobacco cessation programming (e.g., Freedom From Smoking facilitator training). Bilingual: English plus Spanish, Dari, or Pashto. Ability to collaborate effectively across a broad spectrum of backgrounds and perspectives.
