Nurse Practitioner

The Nurse Practitioner collaborates with the Medical Geneticists in providing the highest quality care to patients with genetic disorders and their families.

LOCATION

Shipshewana, IN - The Community Health Clinic, Inc. (CHC) is a 501(c)(3) non-profit healthcare facility that provides excellent and affordable medical care consistent with the needs of the Amish, Mennonite, and rural northern Indiana communities. The CHC serves children and adults with special healthcare needs.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

ADMINISTRATIVE FUNCTIONS

Participates in the clinical supervision of nursing and other clinical staff. Ensures compliance with local, state, and federal regulatory agencies related to clinical services provided by mid-level providers.

PATIENT CARE FUNCTIONS

Performs history and physical examinations on patients with special needs and diagnosed genetic conditions, many of which are inborn errors of metabolism. Consults and collaborates with other members of the health care team to provide acute and ongoing patient care. Assesses patients by using advanced knowledge and skills to identify abnormal findings and diagnose health problems. Assesses and develops care plans, including ongoing monitoring of laboratory studies and adjusting treatment regimens. Contributes to the education of patients, families, community, and team members. Reviews pathways of care, providing input for update or revision as needed. Assist with on-call service to provide 24/7 access to care for patients. This service is split equally between physicians, nurse practitioners, and physician assistants. Evaluates patients’ and families’ development, coping ability, and emotional and social well-being.

OTHER FUNCTIONS

Attends meetings and participates on committees as requested. Participates in CHC research efforts. Reviews current literature and attends training sessions and seminars to keep informed of new developments in the field. Assist the physicians in grant activities. Participates in educational activities for the clinic as requested. Attends and participates in community events. Performs other related duties and responsibilities as directed.

EDUCATION AND/OR EXPERIENCE

To perform this job successfully, an individual should have the following education and/or experience. Master’s degree in nursing (from an accredited program), Family Nurse Practitioner or Pediatric Nurse Practitioner Programs Indiana license/Certification as a Nurse Practitioner and Registered Nurse Experience or interest in treating pediatric and adult patients with genetic disorders, including inborn errors of metabolism. Interest and/or experience in working with the Amish/Mennonite population. Three plus years of patient care working experience as a Nurse Practitioner.

Knowledge of the principles and skills needed to provide patient care and treatment. Knowledge and skill of examination, diagnostic, and treatment room procedures and equipment. Knowledge of medications and their effects on patients. Knowledge of patient education principles. Ability to read and interpret documents such as professional journals, safety rules, operating and maintenance instructions, procedure manuals, and government regulations. Ability to write routine reports and correspondence. Ability to communicate with a variety of people on a one-on-one basis, in meetings, and in group presentations; must be able to work with ill, emotionally upset, and sometimes hostile people. Ability to anticipate and react calmly to emergency situations. Ability to work with mathematical concepts such as probability and statistical inference; ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. Ability to think logically to troubleshoot, analyze situations, and make appropriate decisions. Proficient computer skills, including working knowledge of Microsoft Office Suite, e-mail systems, electronic medical records, and web-based programs. Ability to handle multiple tasks simultaneously.
