Scheduler: Home Health & Hospice Agency
Visiting Nurse & Hospice of Litchfield County is seeking a highly organized, dependable, compassionate full-time Scheduler , preferably with home health scheduling experience to join our Home Health & Hospice team. The Scheduler plays an essential role in coordinating patient visits and supporting our clinical staff to ensure patients receive timely, consistent care.
Responsibilities:
Coordinate daily schedules for nurses, aides, therapists, and other clinical staff based on patient needs, staff availability, and geographic location.
Schedule admissions, routine visits, and follow-up care.
Communicate schedule changes with patients, families, clinicians, and leadership.
Monitor schedules throughout the day and adjust as patient needs and staffing change.
Assist with weekend, holiday, and on-call coverage.
Maintain accurate scheduling information in the electronic medical record (EMR).
Track missed, cancelled, and rescheduled visits.
Assist with entering referrals and coordinating onboarding of new patients.
Provide professional and compassionate customer service to patients, families, and staff.
Qualifications:
Healthcare, home care, hospice, medical office, or scheduling experience preferred.
Excellent organizational, communication, and time-management skills.
Strong attention to detail and ability to manage multiple priorities.
Comfortable working in a fast-paced environment where schedules can change quickly.
Strong computer skills and ability to learn new systems.
Ability to work independently while being a collaborative team member.
Full-time, Monday through Friday position offering a competitive salary and comprehensive benefit package. Coverage for weekend & holidays needed.
