Registered Nurse - Hospice Care
Location: Delaware and Central/South Philadelphia
Assignment Duration: This is a 13-week contract position.
Schedule: AM shifts, 5 days a week (Monday to Friday)
Job Description: We are seeking an experienced Registered Nurse (RN) specializing in hospice care. The ideal candidate should possess at least one year of hospice experience and be familiar with Home Care Home Base (HCHB) software. This role requires a compassionate approach to patient care, providing support to patients and their families in Delaware and Central/South Philadelphia.
Responsibilities
- Conduct comprehensive patient assessments.
- Develop and implement patient care plans in collaboration with the healthcare team.
- Administer medications and treatments as prescribed.
- Maintain accurate and timely documentation of patient visits in HCHB.
- Coordinate care with interdisciplinary team members.
- Provide education and support to patients and families regarding end-of-life care.
Required Experience / Certifications / Licensure
- Minimum of 1 year of hospice experience.
- Registered Nurse (RN) license in hand.
- Experience with Home Care Home Base (HCHB) preferred.
