Location85 Sangers Ln, Staunton, VA, 24401-6712, United States Base Pay $46,008.53 / Year Employee Type FT Exempt Required Degree 4 Year Degree Manage Others No Valley Community Services Board (VCSB) is seeking a compassionate, organized, and family-centered professional to serve as an Intensive Care Coordination (ICC) Facilitator. This position provides care coordination and facilitation services for children and adolescents with complex behavioral health needs and their families through the evidence-based High Fidelity Wraparound model. The ICC Facilitator partners with youth, families, treatment providers, schools, social services, and community stakeholders to develop and implement coordinated plans of care that are family-driven, youth-guided, strengths-based, and trauma-informed. This position plays a critical role in helping families navigate complex systems while promoting stability, resilience, and positive outcomes. Accrue up to 20 days of paid time off (PTO) per year from year one. PTO is immediately available upon accrual. Participation in the Virginia Retirement System. Care Coordination and Facilitation Coordinate High Fidelity Wraparound services for children and youth with complex behavioral health needs. Facilitate multidisciplinary Child and Family Team meetings to develop, implement, review, and update individualized plans of care. Partner with youth and families to identify strengths, needs, goals, and preferences. Advocate for youth and families across behavioral health, healthcare, educational, social services, and natural support systems. Support access to community-based resources, services, and informal supports. Monitor risks and safety concerns and coordinate with crisis services when appropriate. Manage a caseload of approximately 8–12 youth and families. Implement the ten principles of High Fidelity Wraparound, including family voice and choice, strengths-based planning, collaboration, and community-based service delivery. Service Monitoring and Community Collaboration Monitor service implementation and progress toward identified goals. Maintain regular contact with youth, families, and service providers to address barriers and support successful outcomes. Coordinate crisis prevention and response planning with families and treatment teams. Participate in multidisciplinary meetings, case consultations, and coordination activities. Support transitions between levels of care and continuity of services across systems. Deliver services in homes, schools, community settings, and agency locations as appropriate. Documentation and Professional Practice Complete strengths and needs assessments, service plans, and discharge documentation. Maintain accurate and timely documentation in accordance with High Fidelity Wraparound, DBHDS, Medicaid, and agency requirements. Ensure services reflect person-centered, family-driven, and trauma-informed practices. Maintain compliance with HIPAA, confidentiality requirements, and professional ethical standards. Participate in supervision, coaching, training, and quality assurance activities. Complete annual High Fidelity Wraparound training and quarterly coaching requirements. Requirements Minimum Qualifications Bachelor's degree in Human Services, Social Work, Psychology, Counseling, or a closely related field. Eligibility to meet Qualified Mental Health Professional (QMHP) requirements, as applicable. Minimum of two (2) years of experience working directly with children, adolescents, and families in behavioral health or human services settings. Knowledge of community-based services and systems that support youth and families. Ability to work independently while collaborating effectively with multidisciplinary teams. Valid Virginia driver's license (or ability to obtain within 90 days of hire) and acceptable driving record. Preferred Qualifications Experience providing care coordination, case management, or High Fidelity Wraparound services. Experience working within Medicaid-funded, DMAS, or DBHDS-licensed programs. Experience working with multiple child-serving systems, including schools, DSS, juvenile justice, and behavioral health providers. Experience facilitating multidisciplinary team meetings. Strong organizational skills and attention to detail. Excellent written and verbal communication skills. Ability to effectively engage diverse youth, families, and community partners.
