Job Summary: ATTENDANCE BONUS **

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Meadow Falls of Conneaut, a premier long-term care facility dedicated to providing exceptional care and services to our residents. As an LPN at Meadow Falls of Conneaut, you will have the opportunity to make a meaningful difference in the lives of our residents and their families. Every other weekend and 12 hr shifts.

Responsibilities:

• Provide direct patient care, including administering medications, performing treatments, and assisting with daily living activities

• Develop and implement individualized care plans in collaboration with the interdisciplinary care team

• Monitor and report changes in patient condition to the nursing staff and medical team

• Collaborate with other healthcare professionals to ensure comprehensive care and support

• Document patient information accurately and thoroughly in accordance with facility policies and procedures

• Participate in ongoing education and training to maintain current knowledge and skills

• Demonstrate a commitment to providing excellent customer service and promoting a positive and respectful work environment

Requirements:

• Current LPN license

• Minimum 0 years of experience as an LPN

• Ability to work effectively in a fast-paced environment

• Strong communication and interpersonal skills

• Ability to lift, push, and pull equipment and supplies (up to 50 pounds)
