HEALTH HOMES PROGRAM ASSISTANT - Central Islip, NY

Description

MAJOR RESPONSIBILITIES Provide program support to the Health Home Program by working with the Care Coordinators and Director of Health Homes. DETAILED RESPONSIBILITIES Maintain records within Captain (close members, assign goals, etc.) Create member files Take inventory and order office/kitchen supplies as needed Distribute ordered supplies and restock supplies as required in designated areas Provide office orientation for new staff (introduce to other teams, show/review emergency exits, show/review safety protocol) Send a program welcome letter to new members and closing documents to members who have disenrolled Organize and deliver inter-office mail Send member satisfaction surveys Answering Phones, greeting visitors, Scanning, faxing, and filing as required Ability to lift, stand, bend, and sit to complete needed tasks QUALIFICATIONS: MUST HAVE Computer knowledge, including but not limited to Windows, Microsoft Office Ability to work as part of a team with co-workers and with other colleagues as necessary Cultural sensitivity and the ability to relate/work with diverse groups, community agencies, schools, and the general public Strong organizational skills with a focus on detail Ability to Multitask in face pace environment Ability to follow directions and communicate well (orally and in writing) EDUCATION/TRAINING/EXPERIENCE High School Diploma/GED Plus two years of experience as a secretarial or program assistant. Bi-lingual (Spanish) preferred. Requirements A Valid Driver's License and Insured Vehicle. PPD (TB test required with proof of results; no more than 1 year old)
