Tryfacta is seeking an LPN LTC for our client in Vestal, NY 13850. This is a 16 weeks assignment. If you meet the qualifications listed below and are interested, please apply now!

Position Title: LPN LTC Location: Vestal, NY 13850 Duration: 16 weeks Shift: 5X8 Evening and Nights. Guaranteed Hours: 40 hrs Local and traveler candidates accepted. Local pay: $40/hr. Traveler pay: $45/hr. Auto offer

Responsibilities for this position include, but are not limited to:

• Administer medications and treatments as prescribed.
• Perform safe and accurate medication passes.
• Monitor, assess, and report changes in resident condition.
• Maintain timely and accurate documentation.
• Provide high-quality nursing care in accordance with facility policies.
• Supervise and support CNAs on assigned units.
• Deliver compassionate care to residents, including those with Alzheimer's and dementia.
• Collaborate with physicians and the interdisciplinary healthcare team.

To be considered for this position, you should have:

• Active New York LPN/LVN License (required; no license hits).
• Minimum 1 year of Long-Term Care (LTC) experience; candidates with recent rehab, Med-Surg, or hospital experience will also be considered.
• Current BLS Certification.
• Locals and travelers are accepted.
• Ability to pass a medication administration (med pass) test and clinical observation.
• Strong communication, organizational, and documentation skills.
• Dependable with excellent attendance and punctuality.
• Comfortable working in a fast-paced, team-oriented environment.
• Ability to work every other weekend and maintain a fixed evening shift.
• Experience with PointClickCare (PCC) is preferred but not required.
