Job Title Physical Therapy Assistant (PTA) Location Juneau, Wisconsin Schedule / Job Type Contract | Days | 13-Week Assignment | Skilled Nursing Facility (SNF) Key Responsibilities Deliver physical therapy treatments to residents under the supervision of a licensed Physical Therapist Assist in implementing individualized care plans and documenting patient progress Support residents in achieving functional goals related to mobility, strength, and daily living activities Communicate effectively with the interdisciplinary care team regarding patient status and progress Required Qualifications Active Physical Therapy Assistant (PTA) license in the state of Wisconsin Graduate of an accredited PTA program Current CPR/BLS certification Experience working in a skilled nursing facility or similar long-term care setting Preferred Experience / Skills Prior experience in a SNF or post-acute rehabilitation environment Familiarity with Medicare documentation and compliance requirements Why Join Us Opportunity to make a meaningful impact on patient recovery and quality of life in a supportive care setting Gain valuable contract experience with a defined 13-week placement and a day-shift schedule Concentric
