Hospice Quality Assurance Performance Improvement Coordinator

Hartford Healthcare seeks a Hospice Quality Assurance Performance Improvement Coordinator to lead quality, compliance, and performance initiatives across our hospice programs. In this RN role, you will monitor clinical quality metrics, conduct chart audits, and ensure adherence to regulatory and accreditation standards. You will analyze data, identify trends, and implement evidence‑based performance improvement plans in collaboration with an interdisciplinary team. The coordinator will educate staff on best practices, support survey readiness, and champion a culture of safety, compassion, and continuous improvement for patients and families.
