CASE MANAGER

Shift : Mon‑Fri 8:00 AM – 4:30 PM with holiday rotation and occasional weekend coverage. Department : Care Coordination Compensation : $59,322 – $96,762 per year, based on experience. About the Job MU Healthcare seeks a dynamic and compassionate Case Manager to coordinate care with MDs, nurses, therapists, and other service‑line professionals. The role ensures appropriate utilization of resources, monitors services for timeliness and quality, and collaborates with providers, payers, and legal entities. The position is pivotal in ensuring patient and family needs are met through coordination, discharge planning, and education. Assist in developing patient plans of care with interdisciplinary team; counsel, plan and implement discharge transitions. Arrange/authorize delivery of appropriate services; coordinate care to ensure timely provision and outcome success. Facilitate patient progression along the continuum of care and discharge planning for complex needs. Develop individualized protocol guides by service line, diagnosis, social determinants, or special needs; interact with multidisciplinary team to ensure accurate documentation. Monitor service delivery for timeliness, appropriateness, adherence to plan, quality, and cost; report issues to physicians and leadership. Collaborate with other providers (physicians, home‑care agencies, etc.) to discuss patient needs; participate in building community relationships. Serve as liaison between providers, payer, and physicians; provide ongoing education on documentation and utilization. Counsel patients and families; assess biopsychosocial issues; maintain compliance with mandatory reporting. Work with legal entities inside and outside the organization for the patient's best interests; prepare interrogatories and testimony as needed. Required Qualifications Three (3) years of clinical experience providing knowledge of medical terminology, patient diagnoses, treatment protocols and disease processes. One of the following professional credentials: Master of Social Work degree or Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) licensure by the Missouri State Committee for Social Workers board, Missouri Board of Nursing RN or Nurse Licensure Compact multi‑state RN (must apply under new state of residency within 30 days if state changes), Occupational Therapist (OT) licensure by the Missouri Board of Occupational Therapy or OT Compact multi‑state privilege (must apply under new state of residency within 30 days if state changes), Speech Language Pathologist (SLP) licensure by the Missouri Commission for Speech‑Language Pathologists and Certificate of Clinical Competency (CCC) from ASHA, Physical Therapist (PT) licensure by the Missouri Board of Registration for the Healing Arts or PT Compact multi‑state privilege (must apply under new state of residency within 30 days if state changes), Respiratory Care Practitioner (RCP) or RCP‑T licensure by the Missouri Respiratory Care board and Registered Respiratory Therapist (RRT) certification by NBRC. Preferred Qualifications Case management certification or other related specialty. Experience in case management, social work, or utilization management. Additional license/certification requirements as determined by the hiring department.
