Key responsibilities of the Physical Therapist include:

• Provide therapy services 50% in SNF and 50% in Home Health settings
• Supervise two Assistants during therapy sessions
• Utilize Net Health and HCHB software for documentation and scheduling
• Ensure productivity expectations are met: SNF at 82% and HH at 95%

Requirements of the Physical Therapist include:

• Valid Physical Therapist license in Nebraska
• BLS Certification
• Experience or willingness to train in both SNF and HH settings
