Adult & Youth Case Manager | Mental Health Specialist

Join a compassionate team delivering community-based mental health services to adults and children facing significant challenges. As a Case Manager, you will provide skill training focused on measurable goals, guided by medical necessity and a strengths-based approach. Your role includes collaborating with families and treatment teams to develop client-centered treatment plans, helping clients understand their symptoms, and connecting them with natural community supports. You will also coordinate mental and physical healthcare services, facilitating communication among clients, providers, families, and community partners.

Education: A Bachelor's degree in Social Work, Psychology, or a related field is preferred. Alternatively, an Associate's degree plus two or more years of relevant experience, or a combination of education and experience totaling four or more years, is required.

Experience: Ideally, three to five years of professional experience in mental health services or a related area.

Requirements: Completion of CPST Case Management online training, KHS online training, background checks including KBI, child and adult abuse registries, and motor vehicle screening. Adult Case Management training must be completed within 60 days of employment. A valid driver's license, insurance, and reliable vehicle are necessary.

Skills and Attributes: Strong organizational skills and intermediate proficiency with Microsoft Office are essential. Integrity, compassion, accountability, respect, and excellence in communication and strategic thinking are highly valued. The ability to work autonomously and adapt to changing priorities is important.
