Hospice RN On call

We are committed to supporting patients and families during life’s most meaningful moments with dignity, comfort, and respect.Position Summary: We are seeking a dedicated On-Call Hospice RN to join our team. This role is ideal for a nurse who thrives in a flexible, autonomous environment. The nurse will be on-call for 7-days, followed by 7 days off. During the on-call period, the nurse is responsible for triaging calls, making home visits when necessary, and providing critical support to patients and families during after-hours.KEY RESPONSIBILITIESRespond to patient calls and triage health concerns appropriatelyProvide clinical guidance over the phone or in person as neededCoordinate with physicians and care teams for follow-up or escalationMaintain accurate documentation of patient interactionsSupport emergency response protocols when necessaryProvide education to patients/family members regarding the dying process, plan of care and offers guidance with healthcare decisionsPerform death visit per agency’s policyJOB TYPE: PART-TIMEPAY RATE: 93,000-95,000BENEFITS:401(k)Health insuranceDental insuranceVision insuranceLife insuranceMileage reimbursementPaid orientationPaid vacationPaid sick timeQUALIFICATIONSMust be a graduate from an accredited School of Nursing.Must be licensed in NJ as a Registered Nurse by New Jersey State Board of Nursing inaccordance with NJSA 45:11-23 et seq and NJAC 13:37.One year of RN experience. Hospice experience preferred.Must have excellent assessment skills and problem-solving skills.Should be skillful in organization and in the principles of time management and have knowledge of case management processes.Must be a licensed driver with an insured automobile in good working order.The ability to communicate well, both verbally and in writing.Must have no exceptions to RN license, and be able to administer pharmaceuticals, perform medication reconciliation, and any nursing procedures or special treatments needed within the scope of nursing practice for state.
