Requirements:

• 2 years of experience as a RN
• Qualified applicants MUST have at least 2 years years of experience in Home Health
• Qualified applicants MUST have valid RN license, experience with OASIS, and Homecare Homebase EMR systems
• Valid RN license
• Be willing to obtain Wisconsin licensure

Responsibilities: As a healthcare professional in a clinical setting, you’ll deliver patient-centered care while managing documentation and coordination of services to support successful home health outcomes.

• Provide skilled nursing care as an RN in Home Health, ensuring safe, compassionate visits and timely follow-up for patient needs.

• Utilize OASIS and Homecare Homebase EMR to complete accurate assessments, document care plans, and support compliance with home health regulations.

• Support home health workflows using home health terminology, including patient assessments, care coordination, and ongoing monitoring of outcomes.
