Care Coordination Weekend Supervisor - FT Day Weekends

Key Responsibilities Provide oversight and direction for weekend case management operations, ensuring effective workflow and timely service delivery Serve as a liaison between department leadership, case managers, and support staff to promote collaboration and problem resolution Support leadership in the development, implementation, and refinement of processes aligned with departmental and organizational goals Monitor and evaluate workflows and outcomes to identify opportunities for continuous improvement Assist in implementing operational and process changes to enhance efficiency and quality of care Partner with leadership to develop and maintain training and educational resources that support staff competency and professional growth Ensure adherence to regulatory, accreditation, and legal requirements related to case management practices Provide guidance and support to weekend staff, addressing escalations and operational challenges in real time Perform interval departmental audits, and oversee ongoing processes that support the department’s established performance improvement plan Facilitate mandatory weekend huddles to proactively identify and resolve system-wide barriers Provide direction, assistance, and caseload coverage for the department during staff shortages Qualifications BSN or MSW required Current Georgia Registered Nursing (RN) license required if BSN CCM, LMSW, or LCSW preferred Minimum of 3 years of experience in case management, social work, or care coordination Strong knowledge of regulatory and compliance standards in healthcare Excellent communication, collaboration, and problem‑solving skills Ability to lead in a fast‑paced, dynamic environment with minimal supervision Ability to effectively manage multiple tasks, activities, and responsibilities Must work every Friday, Saturday, or Sunday (or Saturday, Sunday, Monday) as required Core Competencies Patient‑Centered Care – Demonstrates a commitment to delivering safe, compassionate, and high‑quality care that prioritizes the well‑being and satisfaction of patients and their families.
